Abstract   The survival of the patients with stage IV gastric cancer will depend on the various prognostic factors. A clinical and statistical analysis was performed to identify these factors among 307 cases of stage IV gastric cancer who were discharged from Kyungpook National University Hospital during 5 years from January 1985 to December 1989. There were no statistically significant differences in the median survival time between sexes, between with and without weight loss, hypoalbuminemia, and anemia. The medisn survival time of the patient group who had Virchow's node, or recta1 shelf, or ascites with class V cytology was 147 days contrary to 210 days in the group without these f indings(p = 0.0452). There was statistically significant difference in the median survival time between the patient who revealed distant metastasis on ultrasonography or abdominal CT scan and the patients did not revealed this finding(p=0.0332). The median survival time of the patients between the group who underwent operation and the group who did not undergo operation revealed statistically significant difference(p=0.0336). The median survival time of the patients with 1iver metastasis on surgical finding was l58 days, without liver metastasis 252 days(p=0.0117), with peritoneal seeding 154 days, without peritoneal seeding 267 days(p=0.0001). The median survival time according to the operative procedure were, exploratory laparotomy 129 days, bypass or simple closure of perforation 183 days, gastrectomy 360 days(p<0.0001). There was no significant difference in the median survival time among T4, N2, HO, PO, T4, N3-4, HO, PO and T3, N3-4, HO, PO group. In patients who revealed T4, N2, HO, PO on operative findings, there was a significant difference in the median survival time between the exploratory laparotomy group and gastrectomy group(p = 0.0233).
